If you want true freedom of choice, with the option of accessing high-quality care in-network or out-of-network, then our PPO (Preferred Provider Organization) is the healthcare plan for you and your family. Empire's National PPO is a health plan that allows members to receive in-network care from any of the doctors, hospitals or ancillary providers participating in the BlueCard® PPO program.

PPO Breakdown

Your Doctor & Network	Choose any provider in an extensive national provider network, through local BlueCross and/or BlueShield plans, consisting of 668,000 highly qualified physician and practitioner locations nationwide.    Access to more than 5,700 participating hospitals nationwide.    Do not need to select a Primary Care Physician (PCP).    Direct access to specialists without needing a referral.
Copay	Check your plan design to see if it has in-network copays.    Out-of-network, you pay a deductible and coinsurance and any amount above the allowed amounts for covered services.
Find a Doctor	You can find a provider near your home or work by searching our directory.   Members registered for online services can get customized directions from their home address! Login or register today.
Emergency Care	Always seek emergency care at the nearest hospital or emergency facility.
